BackgroundProphylactic Bilateral Salpingo-Oophorectomy reduces the risk of developing ovarian cancer. However, the psychological mechanisms that may affect post-surgery Quality of Life among patients who underwent PBSO are still largely unknown. Thus, this study aimed at exploring the direct and indirect associations of satisfaction with medical communication and cancer anxiety on post-surgery QoL among women at high risk of developing ovarian cancer.MethodFifty-nine women who underwent PBSO took part in this cross-sectional study, filling out a sociodemographic and clinical questionnaire, a battery of (...) validated psychological measures and an ad hoc developed scale for the assessment of cancer anxiety. We first examined the correlations among all variables of interest, and then tested if cancer anxiety mediated the association between satisfaction with medical communication and post-surgery psychological QoL, controlling both for time from surgery and education.ResultsPost-surgery psychological QoL was unrelated from any sociodemographic or clinical variable. Cancer anxiety had a significant direct negative effect on psychological QoL, while satisfaction with medical communication had a significant positive direct effect on it. Finally, cancer anxiety significantly mediated the association between satisfaction with medical communication and post-surgery psychological QoL.DiscussionResults suggest that post-surgery psychological QoL of patients who underwent PBSO may be increased with interventions, delivered in a genetic counselling setting, targeting quality of medical communication and cancer anxiety. : The concepts of intention and intentionality were particularly significant notions within the Christian, Jewish, and Islamic medieval philosophical traditions, and they regained philosophical importance in the twentieth century. The theories of intention and intentionality of the medieval Islamic philosopher and physician Ibn Sina and the phenomenological philosopher and mathematician Edmund Husserl are examined, compared, and contrasted here, showing that Ibn Sina's conception of intention is naturalistic and, in its naturalism, is influenced by the medical professional culture to (...) which Ibn Sina belonged. As well, Husserl's anti-naturalistic conception of intentionality is influenced by his background as a mathematician and by his desire to ground mathematics and the empirical sciences in a truly scientific philosophy. In conclusion, an argument is presented for the superiority of the Husserlian transcendentalist account of intentionality over the Avicennian naturalistic account, on the grounds that the latter falls prey to psychologism and reductionism, the two specters that according to Husserl must haunt all naturalistic accounts of consciousness. Girard is widely and unquestionably recognized as one of the most influential thinkers of our times. Interviews, public interventions, and excerpts of his books are featured quite regularly in Italian national newspapers and (...) magazines, and, as a result, Girard's name is known outside narrow academic circles.2 All his books have been translated into Italian, including several unique titles, which are available neither in French nor in English, and which are collections of previously unpublished occasional papers or essays.3 The reception of his thought has not been confined to any specific disciplinary ambit; it has been discussed, among others, by philosophers, literary critics, social scientists, cognitive scientists, theologians, and psychoanalysts.4 This article aims at providing a chronology of Girard's reception in Italy, while surveying the critical literature and the intellectual response to his theory, from the publication of his first book to the present. [End Page 139]From the 1960s to the 1980s: Lies and Truth (About Girard) and the Early Reception of His BooksThe first Italian translation of Mensonge romantique et vérité romanesque5 was released by Bompiani in 1965, with a title that was both misleading and symptomatic: Struttura e personaggi del romanzo moderno (Structures and Characters in the Modern Novel). The reference to structuralism, which was the theoretical fashion of the day in Italian literary circles in the mid-1960s, and the pigeonholing of Girard's exegetical scrutiny as novelistic criticism testify emblematically to the difficulty of classification, plus the substantial misunderstanding that the book withstood both in Italy and in other parts of Europe at that time. To omit mensonge and vérité from the title of the Italian edition is to ignore the "truth" of the matter, that the book was also a mediation on the structure of human desire, extending outside the limits of representation and literature, branching out into a theory of human behavior and the social dynamic that had a profound psychological, anthropological, and sociological reach. Moreover, Mensonge romantique is a meditation on modernity (the novel being its most emblematic form of representation) and the consequences of the unleashing of increasing levels of "internal mediation" within bourgeois society. The reception of this volume was quite cold, and it was reprinted only in 1981, after the wave of discussion stirred by the publication of the Italian translation of Violence and the Sacred. The only review published between 1965 and 1979, by the literary critic Enzo Golino, appeared in 1966 in L'Espresso,6 a weekly magazine that has played, and still plays today, a determinant role in the presentation of Girardian texts to the general public in Italy.Given this minimal reception, it is not surprising that the publication of Girard's groundbreaking work, La Violence et le sacré, in 1972,7 was largely ignored in Italy, in spite of the discussion it stirred in French intellectual quarters and the cultural exchange that Italian intellectuals have always maintained with Parisian cultural circles. The only review that appeared in Italy was by Paola Daniela Giovannelli, a literary critic and an expert on Italian theater, showing that the new work clearly still suffered from the same kind of disciplinary classification and reception as Mensonge romantique. BackgroundThe psychological burden possibly deriving from not immediately undergoing radical treatment for prostate cancer could be a potential disadvantage of active surveillance, especially in the eve of some relevant clinical exams [i.e., re-biopsy, prostate-specific antigen test, and medical examination]. Even if it is known from the literature that the majority of PCa men in AS do not report heightened anxiety, there is a minority of patients who show clinically significant levels of anxiety after diagnosis. The present study aimed to investigate (...) if demographic, clinical, and psychological variables at the entrance in AS were associated with the risk of developing clinically significant PCa-related anxiety 2 months before the first re-biopsy and to offer psychological support to improve quality of life.Materials and MethodsA total of 236 patients participated in the PCa Research International: AS protocol and in PRIAS-QoL study. Demographic/clinical features, health-related QoL domains, coping with cancer, PCa-related anxiety [Memorial Anxiety Scale for PCa ], personality traits, and decision-making-related factors were assessed at T0. MAX-PC was also administered at T1. PCa-related anxiety at T1 was considered to be of clinical significance if the MAX-PC score was ≥1.5. Multivariable logistic regression coupled to bootstrap was used to detect factors associated with high levels of anxiety.ResultsThe median age was 64.4 years. Fifty-six patients reported MAX-PC total score above the cutoff. Three factors were associated with a high level of PCa anxiety at T1: anxious preoccupation [odds ratio = 4.36], extraversion, and prostate-related symptoms. Physical well-being was associated with a low PCa anxiety subscale ; neuroticism and functional well-being were associated with PSA anxiety. Neuroticism and helplessness/hopelessness were associated with fear of progression.ConclusionOnly a partial portion of the sample experienced significant levels of anxiety after 10 months. Psychological assessment should be routinely conducted to detect risk factors for increased anxiety, offering tailored psychological interventions aimed at promoting interpersonal awareness and emotional well-being. It explores the interaction and development of different 'disciplinary' approaches to memory in Ancient Greece, which will enable a fuller and deeper understanding of the whole phenomenon, and of its specific manifestations. We invited five Cavell scholars to write on this topic. What follows is a vibrant exchange among Paola Marrati, Andrew Norris, Jörg Volbers, Cary Wolfe and Thomas Dumm addressing the question whether, in the contemporary political context, Cavell’s skepticism and his Emersonian perfectionism amount to a politics at all.

We argue that the health sciences make causal claims on the basis of evidence both of physical mechanisms, and of probabilistic dependencies. Consequently, an analysis of causality solely in terms of physical mechanisms or solely in terms of probabilistic relationships, does not do justice to the causal claims of these sciences. Yet there seems to be a single relation of cause in these sciences - pluralism about causality will not do either. Instead, we maintain, the health sciences require a theory (...) of causality that unifies its mechanistic and probabilistic aspects. The anti-causal prophecies of last century have been disproved. Causality is neither a ‘relic of a bygone’ nor ‘another fetish of modern science’; it still occupies a large part of the current debate in philosophy and the sciences. This investigation into causal modelling presents the rationale of causality, i.e. the notion that guides causal reasoning in causal modelling. It is argued that causal models are regimented by a rationale of variation, nor of regularity neither invariance, thus breaking down the dominant (...) Human paradigm. The notion of variation is shown to be embedded in the scheme of reasoning behind various causal models: e.g. Rubin’s model, contingency tables, and multilevel analysis. It is also shown to be latent – yet fundamental – in many philosophical accounts. Moreover, it has significant consequences for methodological issues: the warranty of the causal interpretation of causal models, the levels of causation, the characterisation of mechanisms, and the interpretation of probability. This book offers a novel philosophical and methodological approach to causal reasoning in causal modelling and provides the reader with the tools to be up to date about various issues causality rises in social science. From an institutional perspective we contribute to corporate governance of firms by (1) proposing a procedurally fair mechanism that is ethically desirable, and (2) experimentally testing whether procedural fairness crowds-in ethical behavior of managers (on behalf of shareholders) and workers. The experiment sees one ‘manager’ and three ‘workers’ (possibly representing three sections of the firm) co-determining an efficiency-enhancing investment which could harm some workers. Firstly, the manager claims a share of the investment surplus, then workers ‘bid’ for the investment to (...) express their willingness to satisfy the manager’s claim in case the investment is implemented. If the sum of workers’ bids is less than the manager’s claim, the investment is not implementable, which means its surplus will be lost. Workers’ behavior is ‘ethical’ when they veto unfair managerial claims, because the workers have to sacrifice own earnings. Hence, a manager’s fair claim is the ethical response to the threat of workers’ veto. If the manager claims fairly, workers’ ethical behavior is to ‘truthfully’ bid their investment evaluations; by all doing so, they equally share whatever surplus the manager has left for them. The experimental results show ethical behavior of managers in the form of fair claims. Despite these fair claims, workers behave less ethically by strategically underbidding. So the procedurally fair mechanism only partially crowds-in ethical behavior. This study should interest theorists of stakeholder management, especially those engaged in designing the rules of corporate governance. (shrink)

The Thing about Thugs (2010) a novel by Tabish Khair set between London and the Indian region of Bihar, the end of 1830’s and contemporary age, reflects on the relationship regarding the transmission of knowledge and the construction of the colonial criminal. I investigate how Khair, in his neo-Victorian and postcolonial novel, recalls canonical works of Victorian Literature in which thugs, together with other spectral, haunting figures, enter British territory to tell a different version of the official stories, to change (...) the course of the events and uncover truths regarding things happened overseas, during British colonial adventures. Starting from Meadows Taylor’s Confessions of a Thug, to which the novel is strictly connected, it will be interesting to follow Khair’s itinerary across the different narratives of the Thug’s symbolic, political and social meanings in mid- and late Victorian times, from Dickens’s The Mystery of Edwin Drood to Wilkie Collins’s The Moonstone and Conan Doyle’s The Sign of Four and The Mystery of Cloomber. My aim in this article is to investigate the epistemic dimension of reasonableness. In the last decades, the concept of reasonableness has been deeply analysed, and yet, I maintain that a strictly epistemic analysis of reasonableness is still lacking. The goal of this article is to clarify which epistemic features characterize reasonableness as one of the fundamental virtues in the political domain. In order to justify political liberalism through a public justification that averts the risk of falling into a dilemma, (...) it is important to establish in advance which are the epistemic constraints that should be respected. My proposal is that a moderate approach in moral epistemology expresses the best scheme available to us – as moral agents constrained by the limit of our rationality – for establishing a normatively binding, and yet realistic, procedure of justification for political institutions and practices. The ethical monitoring of brain-machine interfaces (BMIs) is discussed in connection with the potential impact of BMIs on distinguishing traits of persons, changes of personal identity, and threats to personal autonomy. It is pointed out that philosophical analyses of personhood are conducive to isolating an initial thematic framework for this ethical monitoring problem, but a contextual refinement of this initial framework depends on applied ethics analyses of current BMI models and empirical case-studies. Cancer diagnosed during pregnancy is a rare event. The most common type of malignancy diagnosed in pregnant women is breast cancer, whose incidence is expected to raise in the next future due to delayed childbirth, as well as to the increased occurrence of the disease at young age. Pregnant women diagnosed with breast cancer are exposed to multiple sources of stress, which may lead to poorer obstetric outcomes, such as preterm birth and low birth weight. In addition, pregnancy involves physiological (...) changes in the breasts that may blur the signs of cancer, with delayed diagnosis and poor prognosis. However, the lived experience of these women was investigated in very few studies. Given this scenario, we conducted this qualitative study to describe and understand women’s subjective experience of being diagnosed with breast cancer during pregnancy. The study was conducted following the principles of Interpretative Phenomenological Analysis. Participants were five women with breast cancer diagnosed during pregnancy, purposefully recruited at a public hospital during medical visits and interviewed at treatment initiation. The interview transcripts were analyzed using thematic analysis. The textual analysis led to the identification of three main themes related to: the emotional storm experienced after cancer diagnosis, and the importance of receiving appropriate information and being focused on treatment decisions; physical changes and comparisons with healthy women, associated with feelings of sadness and inadequacy; being positive, feeling free to disclose all kinds of emotions, religion and spirituality as sources of strength. The paradoxical coexistence of pregnancy and cancer represents a stressful experience for women and their loved ones. Adopting a systemic perspective may be important to understand the effects of such a complex condition, also considering its impact on healthcare workers. (shrink)
